Whole Person Care (WPC) Administration

Your financial contribution helps us provide wraparound services to individuals (youth and adults) giving them access to supportive care, education, and employment services. Your donation will help 40 at-risk community members address their complex needs and transform their life.

These unrestricted funds will be used to provide comprehensive services via shared programming including amenities, barrier removal, behavioral health, food, beverage, childcare, client supportive care, legal services, medical, rental assistance, transportation, etc. These unrestricted funds will be used to provide:

• Intake and triage: The services provided include community engagement activities between partners and government agencies to reach the target population and prepare them to access services after completing preliminary interviews and client needs assessment. The activities are performed by an Admissions Coordinator and Outreach Specialist. The cost per person is $576 x 40 people = $23,040.
• Affordable housing assistance: The services provided include provide referral services to housing agencies and authorities to secure emergency shelter, temporary housing, rental assistance, and vouchers to help participants obtain secure living accommodation and arrangements while in the program and to rapidly transition them to permanent housing. These activities are performed by an Affordable Housing Specialist. The cost per person is $288 x 40 people = $11,520.
• Service coordination: The services provided include multiple types of indirect and direct services to support system navigation for every participant across multiple services and include barrier removal, client supportive services, transportation, behavioral health, medical, substance use disorder, etc. These activities are performed by a Systems Navigator. The cost per person is $360 x 40 people = $14,400.
• Pre-apprenticeship training: The services provided include job readiness training across five high demand industry business, education, healthcare, hospitality, human resources, and engineering and technology. Participants complete one of 36 entry-level job opportunities with employers while completing immersive work-study experiences. These activities are performed by Flexible Facilitators. The cost per person is $360 x 40 people = $14,400.
• Contingent Worker Trainee Stipends: This is a work-study (learn-earn) performance incentive provided to every participant. The cost per person is $484 x 40 people = $19,360.
• Job placement: The services provided include resume writing, mock interviewing, portfolio development, salary negotiations, and direct and indirect links to available jobs in high-demand industry sectors with employer partners. These activities are performed by a Career Specialist. The cost per person is $144 x 40 = $5,760.
• Career advancement: The services provided include professional development plan (PDP), 360-degree feedback evaluation, and client follow up after new hire and placement with employer partners. These activities are performed by a Career Specialist. The cost per person is $144 x 40 = $5,760.
• Data analytics and reporting: The services provided include tracking, managing, and creating reports to show outputs and outcomes at the end of each month, quarter, and year. These activities are performed by a Data Architect. The cost per person is $144 x 40 people = $5,760.
