Who are we?

Whole Person Care Clinic (WPCC), formerly Southern California Care Community, is a 501(c)(3) nonprofit organization dedicated to serving as a safety net for the most vulnerable members of our communities impacted by health inequities.

WPCC provides comprehensive, person-centered services throughout Southern California, including Imperial, San Diego, and Los Angeles counties.

In 2025, in response to the growing need for post-hospitalization support services in Northern California, WPCC expanded its Recuperative Care program to Sacramento. This expansion reflects our deep commitment to closing regional gaps in care and ensuring that individuals experiencing homelessness and medical vulnerability have access to safe, supportive environments for recovery.

What We Do

At the Community Care Campus in Sacramento, WPCC provides short-term residential recuperative care for individuals experiencing or at risk of homelessness who no longer require hospitalization but still need time and support to heal from illness or injury.

This critical bridge between hospital and housing offers a safe, stable environment for recovery. Clients receive:

  • Ongoing medical monitoring

  • Comprehensive social services
  • Medication management
  • Behavioral health services and therapy
  • Life skills development
  • Enrichment activities
  • Care coordination

At Whole Person Care Clinic, our mission is to provide holistic, compassionate, and accessible care that nurtures the physical, emotional, and social well-being of every individual we serve.

Needs

Whole Person Care Clinic’s current priority needs include:

Food donations (fresh produce, pantry staples) to ensure residents have consistent, nutritious meals

In-kind donations such as hygiene products, bedding, clothing, household supplies

Financial support for renovations to make the facility disability friendly.

Resources to create a more calming, trauma-informed environment through improved lighting, furnishings, and wellness spaces

Monetary contributions to sustain daily operations, staffing, emergency assistance, and program services
