The Sierra Community Medical Foundation (SCMF) serves Placer, Nevada, Yuba, Sutter, Colusa, and Sacramento counties by increasing access to health education and promoting healthier lifestyles, particularly for at-risk, underserved, and underinsured community members. Born from the Placer-Nevada County and Yuba-Sutter-Colusa Medical Societies, SCMF was founded to address gaps in care and is driven by local physicians committed to improving community health.

SCMF was created in light of a need for better care for our community.  SCMF primarily focuses on Adverse Childhood Experiences, or ACEs and the relationship to drug use in our community as well as improving the delivery of care to patients through our Physician Wellness Program. Physicians must be well in order for them to provide inclusive, comprehensive care.  SCMF has a Prescription Drug Safety Coalition with the primary goal of addressing the impact of opioid and fentanyl use in our community and schools.  All of our programs are closely intertwined through the common goal of improving the health of our community and quality of life.  

What makes SCMF unique to our community?  We provide and promote programs to assist in public health crises and we collaborate across populations within our communities. 


Mission

Sierra Community Medical Foundation (SCMF) was established to meet a growing need for stronger, more connected healthcare support within our community. Born out of the Placer-Nevada County Medical Society, SCMF is guided by the dedication and compassion of local physicians committed to addressing gaps in care and improving access to vital health resources.

Before the foundation was formally created, community physicians and partners came together to launch an opioid safety coalition serving Placer, Nevada and Sacramento Counties, responding to the ongoing substance use crisis affecting local families. That collaborative effort became the catalyst for a broader mission — bringing together healthcare professionals, community organizations, and residents to tackle complex health challenges through education, prevention, and community-driven solutions.

Today, SCMF continues to build healthier communities by addressing critical issues such as substance use prevention, adverse childhood experiences, and access to wellness resources for families across our region.

Needs

The Sierra Community Medical Foundation (SCMF) seeks unrestricted funding to support programs and initiatives that address urgent and emerging health needs across our region. Flexible funding is essential to allow SCMF to respond quickly and strategically where resources are needed most, ensuring meaningful and measurable impact within the communities we serve.

Unrestricted funds directly support the following priority initiatives:

1. Adverse Childhood Experiences (ACEs) & Social Determinants of Health (SDOH)

SCMF works to reduce the long-term health impacts of Adverse Childhood Experiences by addressing root causes linked to social determinants of health, including access to healthcare, education, housing stability, and food security. Through coalition-building, education, and community partnerships, we promote trauma-informed care, strengthen resilience, and improve outcomes for children, adolescents, and families.

2. Opioid & Fentanyl Safety Coalition

In response to rising opioid and fentanyl-related harm, SCMF leads collaborative prevention and education efforts focused on awareness, harm reduction, and community safety. Funding supports outreach, educational programming, resource distribution, and partnerships that help prevent overdose and support individuals and families affected by substance use.

3. Healthy Families Fitness Festival & Community Wellness Programs

Funding also supports the annual Healthy Families Fitness Festival, a free community event that connects families with health screenings, wellness education, youth sports clinics, nutrition resources, and community services. The festival promotes preventive health, active lifestyles, and access to essential resources, while distributing backpacks and school supplies to children preparing for the academic year.
