Jake's Journey Home is an all-volunteer nonprofit Community Assist Center and mobile outreach entity working with the Folsom, Orangevale, Citrus Heights, Fair Oaks, and outlying communities providing in center-navigational services for our unhoused and at-risk Veterans and community members. Some of the services provided:
Connect. Respect. Assist. Jake's Journey Home assists unhoused Veterans and Community Members out of homelessness and into services to create a lifepath change. From out on the trail to into the house and everything in between.
Jake's Journey Home need has been met. We opened out first Community Assist Center and it is doing amazing! We continue the mobile outreach and now have a brick and mortar to offer more substantial services. We look for more support for this center and another new center.
We are committed to advancing equity by addressing systemic barriers and creating inclusive solutions for unhoused veterans and community members. Our work prioritizes dignity, respect, and access to resources, ensuring that everyone we serve has the opportunity to thrive regardless of their background or circumstances.
