Jake's Journey Home is an all-volunteer nonprofit working out of Folsom, providing navigational services for our unhoused and at-risk Veterans and community members. Some of the services provided:

  • alcohol/drug services, rehabilitation, inpatient
  • mental health services,
  • general humanitarian assistance and referrals as needed
  • disability filing and social security benefits
  • veteran’s benefits and DD214 application
  • Engage the local faith-based services for spiritual help.
  • Seeking out much needed medical help.
  • Provide assistance with Court filings and Criminal Record Expungements
  • Basic Needs (DMV voucher, Social Security Cards, MediCal, DentiCal), clothing, phones, EBT
  • Provide donations to other nonprofits at Veteran Stand Downs
  • Work hand in hand with local nonprofits to provide a full circle of inclusive services
  • Assist the underrepresented, fragile, and special needs persons in all above areas.
  • Work with local, state, federal entities to provide services
  • Educational/Vocational Services/Financial Aid Assistance
  • Bicycle/Trailer Program
  • 211 resource program

Mission

Connect. Respect. Assist. Jake's Journey Home assists unhoused Veterans and Community Members out of homelessness and into services to create a lifepath change. From out on the trail to into the house and everything in between.

Needs

Jake's Journey Home aims to establish a permanent version of their mobile day center to support unhoused individuals transitioning towards a brighter, more successful future.
