Elevated Health and Community is an IRS-approved 501c (3) nonprofit organization with the mission to revolutionize health equity solutions for marginalized communities. We take a community-based approach to health solutions by equipping individuals with the access and resources they need to make informed, high-quality health decisions. We cultivate individual and community health by providing workshops and events which enhance mental and physical health, bolster overall well-being and provide safe outdoor experiences. Through our events and collaborative partnerships with healthcare organizations, service providers, community groups and fitness/outdoor brands, our vision is to transform health solutions for underserved communities nationwide, shifting from a "sick care" model to one that prioritizes overall health and well-being.

Mission

Elevated Health and Community bridges the gap between healthcare systems and the communities they were built to serve. Our mission is to disrupt health disparities and build a more human-centered healthcare system by strengthening patient advocacy, creating inclusive community health experiences, and building an infrastructure founded on equitable, human-centered care for all.

Needs

Elevated Health and Community is actively seeking funding, partnerships, and contracts to formalize and scale our patient advocacy work and community health programming. Near-term priorities include support for PAPA program development and pilot implementation, part-time staffing, community health events, and the operational infrastructure needed to grow sustainably.

We welcome foundation and government grants, individual donations, event sponsorships, and contracts with clinical providers, FQHCs, managed care organizations, and healthcare insurers. We are also actively recruiting mission-driven board members who bring expertise, networks, and leadership capacity to help strengthen and grow the organization. In-kind support and volunteers with experience in community health, patient advocacy, nonprofit operations, or healthcare administration are also valued.

Equity Statement

Equitable care delivery is not achieved simply by making care available. Performance-based health equity requires building the conditions where people can understand their options, advocate for themselves, and trust the systems designed to serve them. At Elevated Health and Community, equitable care is the foundation of every decision we make, every partnership we pursue, and every community we enter. It is not a program feature or a talking point.

While our work supports all communities, we recognize and have a priority focus on communities that have been historically marginalized, underserviced, and systemically deprioritized because of their race, income, language, chronic illness, and barriers to care. High quality healthcare is a right, not a privilege, and our work is designed to address disparities by cultivating system trust, empowering self-advocacy, and mitigating the factors that lead to inequities in care.​​​​​​​​​​​​​​​​
