For over fifty years, CommuniCare Health Centers has served as a community leader providing high-quality, accessible healthcare services for any person who comes through our doors, regardless of health coverage or ability to pay. As CommuniCare+OLE, we continue to provide patient-centered care for 1 in 9 vulnerable Yolo County residents, including primary medical care, dental services, pre- and post-natal services, behavioral healthcare, substance use treatment, and health education.

In the Yolo County community, many residents know us by our health center location names: Davis Community Clinic, Hansen Family Health Center, the Family Wellness Center, Salud Clinic, and the Vida Family Health Center. CommuniCare+OLE also operates a Mental Health Navigation Center to help individuals exiting the justice system and delivers health education and outreach services in locations such as the county jail, schools, and migrant camps.

Our focus on building supportive, culturally sensitive, and collaborative relationships with our patients, along dedicated community outreach and mobile medicine programs, has allowed us to expand access to care for underserved community members. In 2023, our number of patient visits increased by close to 10% to over 136,000. We served 3,800 agricultural workers and their families, many of whom self-reported as seasonal or migrant workers. Of patients who reported their income, nearly 85% live at 100% of the federal poverty level or below - $30,000 for a family of four.

Without community support, CommuniCare+OLE's life saving work in the community would not be possible. Thank you for helping us ensure that comprehensive, high quality healthcare will always be available to everyone who needs it.

Giving Activity

Mission

*CommuniCare+OLE builds stronger communities by ensuring high quality, comprehensive and personalized care for everyone who lives and works in the places we call home.

*We deliver health care in a safe, welcoming environment emphasizing patient partnership, wellness and preventive health in all of our services.

*We invest in employees, learners and volunteers at all levels of the organization.

Needs

CommuniCare+OLE relies on their donors to ensure that we can continue our vital work and fight the health disparities facing our community's most vulnerable. While we are federally funded as a Community Health Center and Migrant Health Center, the real cost of providing comprehensive care to our patients vastly exceeds this level of funding, and the gap grows wider every year. Our wraparound services, including programs aimed at increasing access to care and combating health disparities, are a lifeline for our patients, but many are not covered at all.

Your philanthropy has a direct impact on the lives and community we serve, allowing us to go far beyond the bare minimum of care to improve outcomes and provide a welcoming medical home no matter the circumstance.

Urgent funding priorities include:

• Smile Savers program, which provides in-school oral health education and basic dental services to children, with the goal of preventing costly and irreversible tooth and gum disease in adulthood.

• Mobile Medicine, serving unhoused individuals and seasonal agricultural workers. The van operates 12 hours a week rotating between Davis, Woodland, West Sacramento, and the Capay Valley.

• Food As Medicine program, providing nutrition education and empowers patients to grow, access, and prepare healthy food in our clinic-based garden as part of our chronic disease management and prevention efforts.

• Perinatal program, which includes comprehensive services for prenatal and postpartum care; and provides free of charge car seats, baby carriers, and baby wraps to new parents.

• Primary care services for uninsured patients.

In-Kind Donations

CommuniCare+OLE also appreciates and accepts many types of donated goods, including new, in the box infant car seats or pregnancy support garments/ belly bands (new or clean/used), which we provide to low-income families in our perinatal program. Please contact us at 530-603-7112 or donate@communicareole.org to arrange for dropping off donated items.

Volunteers

The contributions of volunteers - health care providers, community volunteers, and learners such as medical residents, interns, externs, and students - have been essential since our beginnings as an all-volunteer run organization in 1972. If you are interested in donating your time to support the health of our community, please contact us at volunteer@communicarehc.org.

Equity Statement

CommuniCare+OLE strives to address, explore, educate, and respond to the diversity of the human experience. We create an inclusive, anti-racist, equitable, culturally humble, and supportive environment where employees model behavior that enriches our community.
