About Common Good Community

Common Good Community is a community based organization that provides free health coordination services for individuals and families facing complex health and social challenges. Examples of those who qualify for our services include: those impacted by incarceration, individuals experiencing homelessness or housing insecurity, pregnant and postpartum individuals with additional health or social needs, and anyone with multiple chronic conditions.

At Common Good Community, we focus on the whole person. We connect health care with real-life support. Whether it’s finding a doctor, accessing food or housing, navigating the legal system, or simply having someone to talk to, our team is here to help. Through one-on-one guidance from Coordinated Care Companions, access to local resources, and support tailored to your needs, we work together to close care gaps, reduce stress, and build healthier, more connected communities.

 

What We Do

Coordinated Care Companions (CHWs) are at the center of our work, providing HIPAA certified, guided coordination and free, personalized support to individuals and families.

Our Coordinated Care Companions provide support with things such as:

  • Navigating health and social services like housing, food assistance, transportation, social support programs, and therapy

  • Finding health providers that meet your needs and coordinating access to care through referrals, appointment help, and enrollment

  • Signing you up for and helping you understand your benefits

  • Understanding and managing health conditions with education and coaching

  • Advocating for you and your family

  • Creating care plans and providing basic health screenings and assessments (blood pressure, weight, basic needs, stress, etc.)

We believe in whole-person solutions and long-lasting answers, not quick fixes or one and done services. Our Coordinated Care Companions help you navigate a range of holistic services designed to engage, educate, equip, and empower you LONG-TERM. Our services integrate trauma-informed care, community-based outreach, and advanced technology to address social determinants of health, ensuring members receive personalized and coordinated care. Common Good Community helps eliminate barriers for health, and ensures no one falls through the cracks of the medical system.

Specialized Support for Populations of Focus

Common Good Community tailors services to individuals with high healthcare needs, including:

  • Pregnant and postpartum individuals at risk of adverse outcomes

  • Families and youth with complex social and medical needs

  • Individuals experiencing homelessness

  • Adults & children with serious mental illness or substance use disorders

  • Frequent users of emergency services

  • People with multiple or chronic conditions

  • Individuals transitioning from incarceration or institutional settings
