About the Common Good
Common Good Community provides a range of holistic services designed to engage, educate, equip, and empower individuals facing complex health and social challenges. Our services integrate trauma-informed care, community-based outreach, and advanced technology to address social determinants of health, ensuring members receive personalized and coordinated care. By keeping members at the center of care, we can coordinate a hearty future.

Our most specialized service are our Coordinated Care Companions which are our version of Community Health Workers (CHWs). Care companions are HIPAA certified and are trained to provide guided coordination with our members, whether that is planning appointments, health education, or community networking, care companions are there for our members.

Specialized Support for Populations of Focus
Common Good Community tailors services to individuals with high healthcare needs, including:
- Pregnant and postpartum individuals at risk of adverse outcomes
- Families and youth with complex social and medical needs
- Individuals transitioning from incarceration
- High utilizers of healthcare services
