CCAC Vision: Healthy, resilient communities where all people can breathe clean air and thrive.

Now expanding into the Greater Sacramento region, the Central California Asthma Collaborative (CCAC) is proud to serve families in Sacramento, El Dorado, Placer, and Yolo counties, where rising asthma rates, wildfire smoke exposure, and environmental health disparities continue to affect vulnerable communities.

Founded in 2011 as the Fresno/Madera Asthma Coalition, CCAC began with a focus on reducing high asthma rates among low-income children in California’s Central Valley—one of the most polluted and underserved regions in the country. Since then, our work has grown across the San Joaquin Valley, and we are honored to now bring that same commitment to prevention, care, and environmental justice to Sacramento-area families.

At the heart of our work is the belief that clean air is not just a public health goal—it is a human right. We work every day to ensure that families—especially those in historically under-resourced communities—can breathe easier, live healthier, and access the care they deserve.

Through strategic partnerships with managed care health plans, public agencies, and community foundations, CCAC delivers direct services and scalable interventions that improve respiratory health and environmental conditions at both the household and regional levels.

Our flagship initiative, the Comprehensive Asthma Remediation and Education Services (CARES) program, brings asthma care directly into the home. This evidence-based model has helped reduce emergency room visits and improve quality of life for thousands of low-income families. Through CARES, families receive:


  • 🏠 In-home environmental assessments led by certified asthma educators

  • 📦 Delivery and setup of asthma remediation equipment and supplies

  • 💡 Education on proper use of controller and rescue medications, including inhaler technique and monitoring

  • 🩺 Coordination with primary care providers to support clinical care continuity

  • 📋 Ongoing case management by a dedicated team of respiratory therapists and educators


In addition to direct care, CCAC is a regional leader in air quality tracking—supporting community access to real-time air quality data, advancing environmental policy solutions, and uplifting resident voices in environmental health decision-making. We address the root causes of poor air quality, including wildfire smoke, industrial emissions, and long-term environmental injustice.

As we grow our presence in the Sacramento region, we remain committed to culturally responsive, community-centered care. Moments like Earth Day and World Asthma Day shine a light on the urgency of our work—but for CCAC, these days are more than observances. They are reminders of the conditions our communities face year-round, and the resilience they show in the fight for clean air and health equity.

Mission

Advancing health equity and environmental justice by empowering Central California communities through services, research, advocacy, and data-driven solutions to reduce the burden of asthma.

Needs

The Central California Asthma Collaborative (CCAC) was founded in the San Joaquin Valley—one of the most polluted and underserved regions in the country—to address the severe health impacts of poor air quality. For over a decade, CCAC has provided home-based asthma care, education, and environmental health interventions to low-income, rural, and historically underserved communities disproportionately affected by asthma and chronic respiratory illness.

Today, that same need exists in the Greater Sacramento region, where many families face similar challenges—rising asthma rates, wildfire smoke exposure, and barriers to preventive care. Through our Community Asthma Response & Evaluation Services (CARES) program, we are now serving residents across all eight Sacramento-area counties, offering in-home asthma education, environmental assessments, and care coordination to reduce ER visits and improve quality of life.
