Description
The fitness class at Clinica Tepati teaches our patients how easy it is to get some exercise in their daily routine. Whether it be lifting weights with water bottles, stretching with the help of chairs, or creating a walking schedule with the family, our patients learn the importance of keeping active in simple and fun ways for themselves and their family.

-At least 5-10 patients attended each of the 23 classes we held in the 2017-2018 school year.

Description
The CalFresh program at Clinica Tepati is a collaboration project with the River City Food Bank in Sacramento, CA in which we train undergraduate volunteers to help patients sign-up and apply for food assistance. We also educate patients with the eligibility requirements for CalFresh, and provide food accessibility resources for those who are ineligible to receive CalFresh assistance.

-We have counseled about 20-30 patients and had 5-7 patients qualify to receive the benefits of the program.
-Most qualified patients receive on average $100 a month for food assistance.
-Patients learn about how to use their extra money to make smart choices for healthy eating.

Description
The purpose of this program is to be an informative class about Alzheimer's for our patients. We all run the risk of developing Alzheimer's disease, but anyone with uncontrolled diabetes or blood pressure runs a higher risk. Data suggests that our main patient population, Hispanics are at high risk for developing diabetes and high blood pressure putting them at even greater risk for Alzheimer's. We believe patients will benefit greatly in learning more about this disease, especially when considering the specific health concerns our patients are faced with and their relevance to Alzheimer's.

We had an average of 5 patients attend our quarterly Alzheimer's Class in the 2017-2018 academic year. Additionally, we have about 5 patients who are signed up to be regularly contacted for these classes and individual appointments with the specialist to discuss their family members afflicted with the disease.
Description
On select Saturdays, a group of immigration lawyers meet with scheduled patients to alleviate patient fears regarding immigration, as well as provide them with the necessary information to put them on a path towards citizenship. Our patient population often feels uneasy living with tenuous immigration status in this country despite the tremendous work they contribute as working members of our economy. The goal of this service is to inform patients of their rights and provide them with the support to navigate the immigration process and its policies.

- Numerous patients have had their fears alleviated and have received the support and answers they desperately needed.
- Patients have been able to file taxes successfully and apply to programs like Medicaid.
- Most importantly, they now know their rights and can live with a sense of reassurance and security!
Description
We provide one-on-one individualized health coaching to patients who are at risk for developing diabetes, hypertension, or hyperlipidemia or have these diseases and seek to learn how to best live a lifestyle conducive to becoming healthier and living fuller lives. Our health educators are trained on addressing matters related to diet, exercise, and overall well-being. They provide the necessary encouragement to patients to help them live healthy lives. Our patients often keep logs of their diet and exercise and discuss them with our health educators.

- We have seen patients' blood glucose levels decrease drastically!
- Hba1C values have decreased and so have patients' blood pressures.
- Our patients tell their health educators how motivated they now are to exercise and take care of their health for their own sake and for their families.
- Moreover, patients' are much more knowledgeable about how to eat healthy and take care of their bodies.
Description
Clinica Tepati has dramatically improved chronic care for its diabetic patients. An education workshop is held each Saturday at Clínica for our Diabetes patients and is run by a certified diabetes educator, nutritionist, medical student, or doctor. The large number of diabetic patients that came to the clinic brought attention to the need for Diabetes Education, such that the patients could spend time to learn about diabetes, and discuss with other patients how to overcome the many obstacles that come with having diabetes. The Group was started initially as a study, however the possibilities of the program remaining in Clínica is very high. Although the final data needs to be compiled, we have noted that several, and quite possibly a majority, of our patients have benefited greatly from the group, because it has been such a motivating environment for them.

Description
The mental health education initiative seeks to inform patients on the signs and symptoms of depression, as well as the preventative actions available to them if they suspect that they themselves are depressed or believe a friend or family member to be depressed. The objective is to create an atmosphere of openness and remove the perceived stigma of depression. Group classes are conducted on Saturdays and led by mental health professionals and undergraduates. Our volunteers receive rigorous training on how to properly discuss these sensitive health topics with patients, as well as how to conduct group-based psychotherapy. The intention is to assist patients in realizing their ability to effect change in their lives and the lives of those around them in a warm and positive environment that fosters solidarity around an otherwise intimate topic.

- Through our annual, clinic-wide conducted screenings, we were able to identify 75 patients to be treated for a variety of mental health disorders such as depression and anxiety.
- We have expanded on our educational approach to these mental health disorders and hopefully, we continue to reduce the stigmas involved in preventative measures focused on our Latino community.
Description
Many of our diabetic patients require careful monitoring and follow-up, making the need for podiatry specialists imperative. Our podiatry specialists help us provide the best preventative care we can to our most at-risk diabetic patients. Podiatry doctors and students come to Clínica to help patients successfully manage their diabetes and avoid foot-related complications. They teach patients how to inspect their feet daily and maintain proper foot health in light of their diabetes.

- Over 39 different diagnoses have been identified and treated in patients at Clínica Tepati.
- In about 15 Clínica Saturdays, 63 patients make about 100 visits to the podiatric clinic.
- Moreover, a conservative financial value of the services provided in about 15 Saturdays per patient is $275.11.
- Most importantly, patients gained the peace of mind knowing that simple procedures, like debridement, could be done for them, without putting themselves at-risk.

Prescription medications cover a significant portion of our budget every year. We have instituted a 50% copay for prescription medications and try to minimize costs by prescribing generic medications. However, we understand that even the subsidized costs may be a financial burden for our patients so our Pharmacy committee diligently searches for the most affordable prices and is sure to relate the information to our patients.

- Each Saturday, Clinica Tepati spends an average of $227 on medications in order to help our patients purchase what they imperatively need to live their day-to-day lives.
- In the last three years, we have handed out vouchers worth more than $45,000 for patient medications.
- Moreover, our pharmacy committee has signed up over 50 patients on Patient Assistance Programs that subsidize the cost of insulin, test strips, and syringes for diabetic patients who demonstrate sufficient need.
